NORWALK — The following were the results of health department inspections of Norwalk-area food establishments and schools in December 2006:

Friendly Corners Tavern, Ltd.; 21 Mill St., Norwalk; operator, Thomas R. Leto (Dec. 1, regular inspection) No violations. Inspector's comments: Facility is clean.

Cold as Ice Drive Thru; 22 N. Main St., P.O. Box 386, Monroeville; operator, Cynthia Tallman (Dec. 4, regular inspection) No violations. Inspector's comments: Store very clean and in good condition at this time.

The Child Nest; 28 River St., Wakeman; operator, Diana Griffin (Dec. 12, regular inspection) No violations. Inspector's comments: Facility clean and in good condition.

Willard Drive-Thru; 1052 Myrtle Ave., Willard; operator, Jeanne Carmean (Dec. 13, regular inspection) No violations.

Wakeman Mickey Mart Subway; 52 W. Main St., Wakeman; operator, Mickey Mart, Inc. (Dec. 14, regular inspection) No violations. Inspector's comments: Facility clean and in good working condition.

Mercy Hospital, Willard; 110 E. Howard St., Willard; operator, Kyle A. Feasel, CDM, DTR (Dec. 21, regular inspection) No violations.

Dave's Food Mart; 84 Benedict Ave., Norwalk; operator, David Fisher (Dec. 26, regular inspection) No violations. Inspector's comments: Facility is clean at the time of inspection.

Taco Bell; 101 Blossom Center, Willard; operator, Sheila Sherman (Dec. 27, regular inspection) No violations.

K-Mart; 330 Milan Ave., Norwalk; operator, Rodney T. Link (Dec. 28, regular inspection) No violations. Inspector's comments: Store in very good condition at this time. Note: Be sure that any canned items do not have sharp dents on sides of cans or dents on seams. These items shall not be placed on shelves for sale.

Violations

Burger King; 340 Milan Ave., Norwalk; operator, Ronald R. Rybarczyk (Dec. 1, regular inspection) Violation: Clean the food, grease and dirt from under and around the top of fryer hood, around and under equipment in the drive-thru and side of the fryer; Violation: Clean the floor in the mop room of dirt especially around the water heater; Violation: The tiling around the mop drain is missing and cannot be cleaned. Get the tiling and grout repair to make this area easier to clean; Violation: Do not store food (cracker, crouton and canned beans) under the sewage lines in case they drip which could contaminate the food. Note: This facility uses time in lieu of temperature for the chicken fries and ham. If this procedure continues next licensing period, this facility will be licensed as a level four food service due to the food being held without hot holding.

Kiley's; 10 Monroe St., Monroeville; operator, Kiley Faggionato (Dec. 1, regular inspection) Violation: All equipment in a food service must be commercial grade. Replace kitchen microwave oven and toaster with commercial grade units; Violation: Clean the french fry cutter in kitchen of food scraps; Violation: Get sanitizer test kit for three compartment sink in kitchen and bar to regularly check the concentration of the sanitizer; Violation: Get trash containers covered to prevent insect to food contamination in kitchen and bar.

Sheri's Coffee House; 27 Whittlesey Ave., Norwalk; operator, Sheri Thomas (Dec. 6, regular inspection) Violation: All equipment in a food service must be commercial grade. Make sure you replace the equipment with commercial grade units; Violation: Make sure the trash containers are covered in kitchen and service area to prevent insect contamination from the trash to the food.

McDonald's; 31 W. Main St., New London; operator, Steven Payne (Dec. 13, regular inspection) Violation: The concentration of sanitizer (bleach) in the bucket of wiping cloths was zero. Make sure there is at least 50 ppm bleach in the bucket to be able to properly sanitize food surfaces; Violation: The dishwasher has been broken for a week and will not be repaired for two more days. The personnel are hand washing the equipment until the dishwasher is repaired. Get the dishwasher repaired; Violation: Make sure all trash containers in the kitchen area are covered to prevent insect contamination from the trash to the food; Violation: The door to the employees restroom does not self-close. The door must have a self-closing mechanism to prevent insect contamination from the restroom to the food. Note: The sanitizer test kit used to regularly check the concentration of the sanitizer in the dishwasher was velcroed to the wall and the lid to the kit was sealed shut with the velcro indicating the kit is not regularly used. The manager has other test kits in their drawer which can be used until the one on the wall is fixed to allow the lid to be opened.

Miller Brothers Grocery, Inc.; 166 W. Main St., New London; operator, Miller Brothers Grocery, Inc. (Dec. 13, regular inspection) Violation: All equipment in a food service must be commercial grade. When you replace these items, replace them with commercial grade units; Violation: Make sure all trash containers are covered in meat cutting, product and deli areas to prevent insect contamination from the trash to the food. Inspector's comments: Facility is clean.

Mtd. Products; 979 S. Conwell Ave., Willard; operator, 979 S. Conwell Ave., Willard (Dec. 13, regular inspection) Violation: Chili was placed on line for sale (temperature was 121 degrees). Minimum hot holding temperature is 135 degrees. Chili was reheated in microwave to 176 degrees and returned to service line; Violation: Midwest Cafe is reheating some food products. This is not allowed under the level three license they currently hold. Recommend updating to level four at next licensing period.

Corner Store; 2754 Ohio 18 East, Collins; operator, William and Nancy Horn (Dec. 14, regular inspection) Violation: The macaroni and pasta salads in deli were not date label to show the disposal date for the salads. Make sure the salads are dated seven days from the date of preparation for disposal to prevent excessive bacterial growth on foods sold to the public; Violation: The microwave oven, toaster and floor freezer are not commercial grade units. All equipment must be commercial grade. Make sure commercial grade equipment is purchased when replacing these items; Violation: All kitchen trash containers must be covered to prevent insect contamination from the trash to the food; Violation: There is no mop sink at this facility. Make sure a mop sink is installed if this facility is renovated or under new ownership. In the meantime, the wastewater in the mop bucket must be poured down the toilet and into the sewage treatment system.

Fisher-Titus Medical Center; 272 Benedict Ave., Norwalk; operator, John S. Loose (Dec. 15, regular inspection) Violation: The margarine and pop are stored under the dripline to the condenser in backroom large walk-in cooler. Move all food away from the dripline to prevent contamination if the dripline leaks.

New London Pizza House; 2 S. Main St., New London; operator, Larry and Rita Fritz (Dec. 18, regular inspection) Violation: Critical Do not store hoagie buns under raw chicken in walk-in cooler. The chicken will contaminate the buns if it drips onto them. Store ready-to-eat foods over the raw foods (changed during inspection); Violation: Two microwave ovens, two floor freezers and pizza upright freezer are not commercial grade. When these items are replaced, get commercial grade units; Violation: The ventilation hood screens and fire nozzles need cleaned of grease; Violation: Get sanitizer test kit (quat) for the three compartment sink so you can regularly check sanitizer concentrations; Violation: The person-in-charge must be knowledgeable about correct storage separate of raw and ready-to-eat foods as well as food temperatures and three compartment procedures with the sanitize test kit available; Violation: Make sure the floor is regularly cleaned under the pizza press to prevent an insect infestation; Violation: Storeroom do not store insecticide in the food service. Store it away from the food service area to prevent any contamination of the food and/or equipment.

The Carriage House of FTMC; 175 Shady Lane Drive, Norwalk; operator, John S. Loose (Dec. 18, regular inspection) Violation: Make sure all food is stored away or protected from the dripline to the condenser in walk-in cooler and freezer to prevent contamination if the dripline leaks; Violation: All equipment in a food service must be commercial grade. Replace kitchen microwave oven with a commercial grade unit; Violation: The upstairs trash container needs to be covered to prevent insect contamination from the trash to the food.

American Legion 292; 185 N. Main St., New London; operator, American Legion Post 292 (Dec. 19, regular inspection) Violation: Kitchen microwave oven, refrigerator and freezer are not commercial grade units. When this equipment is replaced, make sure it is replaced with commercial grade units; Violation: Get a sanitizer test kit for the three compartment sinks in the kitchen and bars so the concentration of the sanitizer can be checked on a regular basis (quat 200-400 ppm). Inspector's comments: Facility is clean.

The Village Pizza Barn; 53 Park Ave., New London; operator, Nancy C. Tittle (Dec. 19, regular inspection) Violation: Critical In the kitchen reach-in cooler, eggs were stored above ready-to-eat foods, which could be contaminated if the eggs break and leak. Store raw foods under ready-to-eat foods (done during inspection); Violation: Person-in-charge must be knowledgeable of food rules and the proper storage of ready-to-eat and raw food items in the cooler; Violation: The microwave oven, upright and floor freezers are not commercial grade units. When you replace these units, replace them with commercial grade equipment; Violation: Clean the flour around, under, and on the floor of the pizza press; Violation: A quat sanitizer was being used in the three compartment sink with bleach sanitizer. Either get a bleach sanitizer test kit or use quat sanitizer; Violation: The tile flooring on back room floor is missing and makes the floor hard to clean. Get the floor tiling repaired.

M&M Pizza House; 47 Whittlesey Ave., Norwalk; operator, Mark M. Hudson (Dec. 20, regular inspection) Violation: All equipment in a food service must be commercial grade. Make sure when you replace the back room floor and upright freezers they are replaced with commercial grade equipment; Violation: The inside covers to the back room floor freezers are broken on both freezers and could allow insulation to get into the food. Repair the cracks in the door covers to prevent the insulation from coming out; Violation: Get an AASE approved backflow prevention device on the utility sink water faucet to prevent gray water from getting into the public water supply from the sink when there is a hose on it; Violation: Get a cover for the restroom wastebasket to prevent insect contamination from the materials in the wastebasket to the food.

Norwalk Childcare Center, Inc.; 120 Shady Lane Drive, Norwalk; operator, Jan Shingledecker (Dec. 21, regular inspection) Violation: Make sure a hat or hairnet are worn when preparing food to prevent contamination of the food; Violation: Person-in-charge must be knowledgeable of all food rules including the use of hats or hairnets in the kitchen area; Violation: All equipment in a food service area must be commercial grade. When the kitchen upright freezer is replaced, make sure it is replaced with commercial grade units; Violation: Get a lid on the kitchen trash container to prevent fly contamination from the trash to the food.

Subway Sandwiches & Salads; 10 W. Walton St., Willard; operator, William McKelvey (Dec. 21, regular inspection) Violation: Do not store food (onions) in utility room. This room contains cleaning chemicals etc. Also food needs to be stored maximum six inches off floor; Violation: Inside floor of single door refrigerator needs cleaned. Place on routine cleaning schedule to prevent food particle buildup; Violation: Floor is dirty especially in areas around mop boards and back room areas. Please clean.

Domino's Pizza; 215 Myrtle Ave., Willard; operator, Greg Polachek (Dec. 26, regular inspection) Violation: Storage bin where clean utensils are stored needs cleaned. This should be put on a daily cleaning schedule; Violation: Handwash sink next to prep table needs to be resealed to the wall.

Pizza Hut; 188 Milan Ave., Norwalk; operator, Midland Food Services, L.L.C. (Dec. 26, regular inspection) Violation: In dishwashing area, the prewash sprayer is laying below the top level of the sink and could cause any dirty water to be siphoned into the public water system during a fire in the area. Make sure the hose is adjusted to keep the sprayer above the top of the sink; Violation: Clean the can opener of food particles; Violation: Clean the top of the pizza oven of food particles and dirt; Violation: Make sure all kitchen and dishwashing area trash containers that hold food are covered to prevent insect contamination from the trash to the food; Violation: The shelf in the utility room is badly warped and cannot be easily cleaned. Either remove, replace or repair it to make it easier to clean; Violation: Clean the floor under the pizza oven of food scraps. Note: There are several abandoned mouse traps laying on top of the water heater that still have food in them which could cause an insect infestation problem. Either store the traps in a bag or remove the traps from the food service.

Pepperidge Farm Cafeteria; 3320 Ohio 103 East, Willard; operator, Cuyahoga Vending Services, Inc. (Dec. 27, regular inspection) Violation: Please clean food racks and walls of walk-in cooler. This should be placed on a minimum semi-annual cleaning schedule. More often is needed; Violation: Please repair lighting in reach-in McCall refrigeration unit.

Smith's Country Counter of Norwalk, Inc.; 3675 Lehigh Road, Norwalk; operator, Richard J. Smith (Dec. 27, regular inspection) Violation: Make sure that if the buffalo meat is prepared (repackaged and/or cut/portioned) that it has this facility's label on it to identify where the meat was purchased from and what the meat is identified as (done during inspection); Violation: Any loosed wrapped meats such as turkey, chicken, ham or roast beef must be date labeled for seven days from the date of opening it for disposal to ensure that meats are not served that may not be of good quality (done during inspection).

Subway Sandwiches & Salads; 355 U.S. 250 North, Norwalk; operator, Lois Owens (Dec. 27, regular inspection) Violation: The employee wearing a visor has a large amount of hair positioned in front of the visor which could fall into any food that is prepared. Make sure all employees have their hat controlled with hairpins to reduce the chance of contamination of the food. (Gave handout with proper hair control examples); Violation: All kitchen trash containers that are used for waste food must be covered when not in use to prevent insect contamination from the trash to the food; Violation: The handsink in service area was blocked with two Mayo dispensers that had failed during lunch rush hour. The handsink must be left free to be able to use at all times, especially during lunch when the food employees should be washing their hands before putting on a new set of gloves. (Gave handout on proper glove usage.)

Gulf Inn; 1392 U.S. 20 East, Norwalk; operator, Tom Buckingham (Dec. 28, regular inspection) Violation: The food prep cook must wear a hat or hairnet when preparing ready-to-eat foods to prevent contamination of the food; Violation: Person-in-charge must be knowledgeable of all food rules and proper kitchen procedures such as three compartment sink procedures and proper storage of food (raw and ready-to-eat foods); Violation: Critical There was a bag of raw bulk ground pork being stored on top of flour wraps in back room refrigerator. All raw foods must be stored below ready-to-eat foods to prevent contamination of the ready-to-eat foods from the leaking of the raw foods (corrected during inspection); Violation: Microwave oven is not commercial grade. All equipment in a food service must be commercial grade. When this unit is replaced, get a commercial grade unit; Violation: Get a sanitizer test kit (bleach) for three compartment sink so the correct concentration of the sanitizer can be checked (50 ppm bleach); Violation: There is a hole in the wall above the three compartment sink and part of the flooring in the kitchen is missing. Get the hole repaired and new tiling on the floor to make them easier to clean.

Mid American Restaurant, Inc., Jerry's No. 30; 117 E. Walton St., Willard; operator, Mid American Restaurant, Inc. (Dec. 28, regular inspection) Violation: Person-in-charge must be knowledgeable about food rules and procedures; Violation: Gravy prepared in bulk and refrigerated must be date-marked; Violation: All raw meat must be thawed on bottom racks in walk-in. Chicken being on the lowest shelf, then pork, then beef. Raw eggs must also be stored on lowest shelves in reach-in beside grill. Inspector's comments: Facility is in nicer condition today than in previous inspections.

Subway; 271 Sandusky St., Monroeville; operator, Resley and Resley, Inc. (Dec. 28, regular inspection) Violation: Inside top of microwave cavity dirty with cooked/dried food debris. Clean inside of microwave (including top) at least once every 24 hours or as often as necessary to prevent contamination. Inspector's comments: Overall, facility clean and in good condition.

Victory Inn; 6 Myrtle Ave., Willard; operator, Brian Hamman (Dec. 28, regular inspection) Violation: Repair urinal in men's restroom. There is no water at urinal. Note: Refrigerator/freezer unit is not a commercial unit. When this unit fails or is replaced, it must be with a commercial unit.

South Central Elementary/Junior High School; 3291 Greenwich Angling Road, Greenwich; operator, South Central Schools (Dec. 11, regular inspection) No violations.

St. Mary School now Norwalk Catholic Schools-Early Childhood Center; 77 State St., Norwalk; operator, Dan Gerold (Dec. 6, regular inspection) Violation: Replace burnt light bulb in hood system.

Willard Junior High School; 955 S. Main St., Willard; operator, Melodie A. Adelman (Dec. 11, regular inspection) Violation: Person-in-charge must be knowledgeable about food rules and procedures; Violation: Sour cream was out at teachers' counter during entire serving period. This contains milk products and needs to be kept in cooler.

Norwalk Middle School; 64 Christie Ave., Norwalk; operator, Norwalk City Schools (Dec. 14, regular inspection) Violation: Microwave oven not commercial grade. Remove or replace microwave with a unit that has been approved by a recognized food testing agency.

Willard Senior High School; 123 Whisler Drive, Willard; operator, Melodie A. Adelman (Dec. 19, regular inspection) Violation: Food that is for private use may not be stored in a licensed facility. Product in walk-in freezer must be removed.
