The Ohio State Highway Patrol is currently investigating a two-vehicle fatality crash.

The crash occurred at at 4:32 p.m. Monday at the intersection of State Route 590 and County Road 41 in Sandusky County's Jackson Township.

Bedabibhas Mohanty, 36, of 358 Portside Circle in Perrysburg was traveling northbound on State Route 590 in a 2014 Toyota Prius.  Lesley Steward, 27, of 516 Rawson Ave. in Fremont was operating a 2001 Chevrolet Cavalier. Also in Mr. Steward’s vehicle was the right front passenger Angelita A. Zavala, 25, and the rear passenger Audrianna Z. Steward, 2, also of 516 Rawson Avenue in Fremont.

Through the course of the on scene investigation troopers learned that the vehicle driven by Lesley Steward was traveling eastbound on County Road 41 when it approached the stop sign posted at the intersection of State Route 590. According to witnesses Mr. Steward ran the stop sign and was struck by Mr. Mohanty’s vehicle. Both vehicles went off the northeast corner of the intersection prior to final rest.

Angelita A. Zavala was pronounced deceased at the scene. Lesley Steward and Audrianna Steward were flown by LifeFlight to Mercy St. Vincent Medical Center in Toledo. Bedabibhas Mohanty was also flown by LifeFlight to the University of Toledo Medical College. The crash remains under investigation and no charges have been filed at this time.  Alcohol does not appear to be a factor in the crash.  The intersection on State Route 590 and County Road 41 remained closed for approximately two hours while Troopers and emergency personnel were on scene.

The Ohio State Highway Patrol was assisted on scene by the Sandusky County Sheriffs Department, Helena Fire & EMS, and Life Flight.
