Community Center and Village Offices; 48 W. Broadway, Plymouth; operator, Village of Plymouth (June 29, regular inspection) No violations. Inspector's comments: Facility neat and clean.

Violations

Kiley's; 10 Monroe St., Monroeville; operator, Kiley Faggionato (June 1, regular inspection) Violation: The pizza sauce was stored under the drain line for the walk-in cooler evaporator and condensation dripped onto the plastic wrap covering the pizza sauce. Do not store any food under the drain line to prevent contamination from the condensation dripping on the drain line; Violation: The microwave oven, toaster, and upright refrigerators and freezers are not commercial grade. If they are to be used, replace them with commercial grade units that have the approved testing agency seal; Violation: Clean the flour from on and around the pizza press. Clean the food from the fryers; Violation: Get a sanitizer test kit for three compartment sink to regularly check the concentration of the sanitizer in the third sink (wash, rinse and sanitize) (bleach at least 50 ppm); Violation: All trash containers that hold waste foods must have a cover on them when not in use to prevent insect contamination from the waste food to the food; Violation: Get rid of unused equipment that is stored in the abandoned walk-in cooler and in the basement to make the facility easier to clean; Violation: After making the pizza sauce, make sure you date label the sauce for seven days from the date it was prepared for disposal (sauce is prepared with water and spices).

Lazy J RV Resort; 4888 U.S. 20, Bellevue; operator, Paul Hebner (June 1, regular inspection) Violation: Several pieces of equipment in kitchen is not approved by a recognized food equipment testing agency. Replace equipment with units approved by a recognized food testing agency.

Gulf Inn; 1392 U.S. 20 East, Norwalk; operator, Tom Buckingham (June 4, regular inspection) Violation: Upright refrigerator and freezers are not commercial grade. Replace these items with commercial grade units that have an approved testing agency seal; Violation: The back splash for the three compartment sink needs to be either bolted to the wall or sealed with caulk to prevent water from going inside the back of the sink; Violation: Get a back flow prevent device on the mop faucet to prevent wastewater from contaminating the public water system; Violation: The front half of the kitchen floor is bare wood and needs to be tiled to make it easier to keep clean. (Owner is in the process of doing the bar area and then the kitchen.)

McDonald's; 31 W. Main St., New London; operator, Steven Payne (June 4, regular inspection) Violation: Clean the breading in the freezer door gaskets; Violation: Clean the walk-in freezer floor of food. Note: Make the employees aware of the location of the sanitizer test kits for the wiping cloths and the dishwasher and they are aware which test kit goes for each activity. Inspector's comments: Facility and restrooms are clean and in good condition.

Rehab. & Nursing Center at Firelands; 204 W. Main St., New London; operator, Rehab. & Nursing Center at Firelands (June 4, regular inspection) Violation: The microwave oven and toaster are not commercial grade. Replace these items with commercial grade units that have a seal from an approved testing agency. Inspector's comments: Facility is clean and in good working order.

Ernsthausen Aquatic Center; 100 Republic St., Norwalk; operator, City of Norwalk/Ernsthausen (June 5, regular inspection) Violation: All equipment must be commercial grade. Microwave oven and upright freezer must be commercial units with an approved testing agency seal.

Norwalk Childcare Center, Inc.; 120 Shady Lane Drive, Norwalk; operator, Jan Shingledecker (June 5, regular inspection) Violation: Make sure the upright freezer is commercial grade, if it is not, replace it with a commercial grade unit that has a seal from an approved testing agency. Inspector's comments: Facility is clean and in good condition.

Taco Bell; 101 Blossom Center, Willard; operator, Sheila Sherman (June 5, regular inspection) Violation: Ensure that all serving utensils are clean before returning to storage rack; Violation: Containers on storage shelf had water accumulation. Adequate space needs to be provided for all clean pans and utensils to be air dryed prior to returning to storage.

Arby's; 106 E. Walton St., Willard; operator, ARG (June 6, regular inspection) Violation: Make sure that glue from time dots are completely removed from pans during washing; Violation: Food product in prep freezer needs to be covered to help prevent accidental environmental contamination. Inspector's comments: Facility neat and clean at this inspection. Violations from last inspection have been corrected.

Discount Drug Mart; 201 Cline St., Norwalk; operator, Discount Drug Mart (June 6, regular inspection) Violation: Clean the food splashed on the ceiling of the walk-in cooler. Note: Watch the cans placed out for sale. If they are severely dented where the top/bottom or side seals may be damaged, they must be removed from sale. Inspector's comments: Facility is clean and in good condition.

Mercy Hospital, Willard; 110 E. Howard St., Willard; operator, Kyle A. Feasel (June 6, regular inspection) Violation: Clean dust debris from hood system; Violation: Door seal on refrigeration unit behind serving counter needs repaired/replaced.

Becky's Hotdogs; 1300 Ohio 18, Norwalk; operator, Norwalk Raceway Park, Inc. (June 7, regular inspection) Violation: Microwave oven must be commercial grade. Replace this unit with a commercial grade microwave oven that has the approved testing agency seal; Violation: Get hand soap for the handsink so the food employee can properly wash their hands; Violation: Clean the bugs and dirt out of the light shield.

Main Concession; 1300 Ohio 18, Norwalk; operator, Norwalk Raceway Park, Inc. (June 7, regular inspection) Violation: The walk-in cooler and freezer evaporator drain lines were missing. The water must drain out of the coolers and into a drain. Will recheck during the national in three weeks. (June 28, follow-up inspection) Violation corrected. Walk-in cooler and freezer drain lines installed.

Miller Brothers Grocery, Inc.; 166 W. Main St., New London; operator, Miller Brothers Grocery, Inc. (June 7, regular inspection) Violation: In the deli area, there were a couple of loose wrapped lunch meats that need to be date labeled to make sure they are disposed of within seven days. Two self-serve cakes did not have the label for artificial colors on its ingredient list. Make sure it is on to warn anyone allergic to food coloring; Violation: The toaster and microwave oven are not commercial grade. Replace these units with commercial grade equipment that has an approved testing agency seal (Note: The coffee pot is only used for employee use). Inspector's comments: Facility is clean and in good operation condition.

M+M Pizza House; 47 Whittlesey Ave., Norwalk; operator, Mark M. Hudson (June 8, regular inspection) Violation: Make sure the food opened or prepared such as the pizza sauce is date labeled for seven days from the date of preparation for disposal; Violation: The upright freezer is not commercial grade. Replace these items with commercial grade equipment that has an approved testing agency seal.

Marco's Pizza; 105 Whittlesey Ave., Norwalk; operator, North Coast F.H.D., Inc. (June 8, regular inspection) Violation: The upright freezer is not commercial grade. Replace this unit with commercial grade equipment that has an approved testing agency seal; Violation: Front and back doors were left opened during operation. There is a possiblity of dirt, dust and insects contaminating the food from the open door. Either get the air conditioning repaired or install screens for the doors. Do not leave them open without proper screening; Note: Three compartment sink has bleach test kit and bleach available, but there is also quat sanitizer available which does not have a test kit. Make sure you get a quat test kit if you use the sanitabs as your sanitizer. Inspector's comments: Facility is clean.

Uncle Dudley's Restaurant & Catering; 21 E. Maple St., Willard; operator, Mary Lou Haver Bordner (June 8, regular inspection) Violation: Person-in-charge must be knowledgeable about food rules and procedures; Violation: Label working containers of foodstuff in reach-in refrigerator; Violation: Handwashing sink is blocked by mixer. This sink needs to be available for FSO employees to wash hands; Violation: Potatoes that have been cooked for home fries were at 93 degrees. After cooking, potatoes are considered potentially hazardous food and must be maintained at 135 degrees and above or 41 degrees or below; Violation: Foodstuffs in walk-in cooler need to be date-marked with the discard date which is seven days at 41 degrees or below.

Greenwich Little League Concession; 45 Main St., Greenwich; operator, Little League Parents (June 11, regular inspection) Violation: Chest freezer and microwave oven not commercial grade. Replace equipment with units that have been approved by a recognized food testing agency.

Kats Iron Skillet; 2114 U.S. 224 East, Greenwich; operator, William H. Mobley (June 11, regular inspection) Violation: Hot water faucet drips and "S" trap leaks on hand sink in kitchen. Repair leaks in handwash sink faucet and drain; Violation: Inside of prep table compartment accumulating water. Open drain to allow unit to drain condensed water as designed; Violation: Several pieces of equipment not commercial grade (microwave, toaster). Replace with units that have been approved by a recognized food testing agency; Violation: Person-in-charge not ensuring that rules are being followed by allowing equipment to not be maintained in good repair. Ensure that equipment is maintained and rules are followed; Violation: Handle missing from microwave door not allowing it to open properly. Replace microwave with a commercial unit.

Moose Lodge No. 1248; 11 S. Prospect St., Norwalk; operator, Tracy Olden (June 12, regular inspection) Violation: The upright refrigerators, freezer and microwave oven are not commercial grade. When these items are replaced, replace them with commercial grade equipment that has an approved testing agency seal; Violation: Clean the inside of the right refrigerator of spilled food. Clean the inside of the microwave oven of food. Inspector's comments: Facility is clean and in good condition.

New London Recreation Park Concession; Grove Street; operator, Barry and Kathleen Overy (June 12, regular inspection) Violation: The crockpot and toaster as well as backroom freezer are not commercial grade. Replace these items with commercial grade equipment that has an approved testing agency seal; Violation: Chili in crockpot can not be heated in the crockpot. It must first be heated to at least 165 degrees Fahrenheit before placing it in hot hold. Inspector's comments: Facility is clean.

New London Pizza House; 2 S. Main St., New London; operator, Larry and Rita Fritz (June 12, regular inspection) Violation: The two microwave ovens in the kitchen and floor freezer near to backroom slicer are not commercial grade. Replace these with commercial grade equipment that has an approved testing agency seal. Inspector's comments: Facility and restrooms clean.

Stillwater Creek; 31 E. Main St., New London; operator, Christine Ericson (June 12, regular inspection) Violation: Critical The food prep employee was not wearing a hat or hairnet. The food employee must wear a hat or hairnet to prevent contamination of the food; Violation: The person-in-charge must be familiar with and employ food rules and procedures in order for the food service to be able to serve food that is at the correct temperature and not contaminated. Especially watch for food employee use of hat/hairnet, gloves and proper handwashing as well as proper food temperature and prevent from contamination by covering the food when stored in the refrigerators; Violation: Critical Make sure all food is covered in kitchen and backroom coolers when not in use to prevent contamination from upper shelves; Violation: Critical The breading bucket was open allowing for contamination and the container is not labeled to make sure all employees know what is in the container. Get the container closed and properly labeled; Violation: Critical No food stored in the cooler such as the soup was date labeled to show the date for disposal. All food opened or prepared must be date labeled to show the disposal date so no spoiled food is served to the customers; Violation: Critical The tartar sauce was 51 degrees Fahrenheit in some ice. The cole slaw was 61 degrees Fahrenheit not in ice and the eggs were next to the grill at 81 degrees Fahrenheit. All hazardous foods must be kept cold held below at least 44 degrees Fahrenheit to prevent bacterial growth. All food must be kept in the cooler until needed and then placed back into the cooler when done; Violation: The upright refrigerator, toasters, microwave oven and floor freezer are not commercial grade. Replace these items with commercial grade equipment that has the approved testing agency seal; Violation: Critical The handsink was being used to drain noodles. This sink cannot be used for any purpose other than for handwashing. Make sure this facility is adequately supplied with soap and paper towels; Violation: Critical The evaporator drain line on walk-in freezer is draining into the freezer causing a large amount of ice to form inside of the freezer causing a food contamination and safety hazard. Get the drainline fixed to eliminate the ice; Violation: The tiling in dishwashing area is missing on part of the floor and needs to be replaced to make it easier to keep clean; Violation: Critical There was no sanitizer test kit to check the proper concentration of the sanitizer used in the dishwasher. Get a sanitizer test kit to be able to properly check the sanitizer concentration. (June 20, follow-up inspection) This is a reinspection of critical violations found during the previous inspection: 1. The food employees are wearing hats and using gloves and utensils; 2. All food is stored with covers and date labeled for disposal; 3. The breading bucket in the back room is covered and properly labeled; 4. There is no food let out during the inspection. All food is stored in the coolers; 5. The kitchen handsink is accessible and adequately supplied with soap and papertowels; 6. The dishwashing area has a sanitizer test kit (using bleach sanitizer, and the test kit is a bleach test kit); 7. The walk-in freezer's drainline is being repaired. If it cannot be repaired, the owner will be purchasing a chest freezer (make sure the chest freezer is commercial grade with an approved testing agency seal). All critical items have been corrected or are being addressed in the case of the walk-in freezer.

Pine House Pizza Ltd.; 6056 U.S. 20, Wakeman; operator, Steve Good (June 13, regular inspection) Violation: There are upright freezers that are not commercial grade. Replace these items with commercial grade equipment that has an approved testing agency seal. Note: The microwave and a freezer have been replaced with commercial equipment since last inspection. Owner will have the facility completely commercial grade by next year.

MTD Products; 979 S. Conwell Ave., Willard; operator, AVI Food Systems, Inc. (June 14, regular inspection) Violation: Person-in-charge must be knowledgeable about food rules and procedures; Violation: Product in Delfield Refrigerator is not at 41 degrees or below today. Call maintenance to get unit checked as soon as possible; Violation: Nozzles on soda fountain machine have syrup buildup. Place on a daily cleaning schedule.

American Legion No. 292; 185 N. Main St., New London; operator, American Legion Post 292 (June 15, regular inspection) Violation: The microwave oven, floor freezer, upright refrigerator and freezer are not commercial grade. Replace these items with commercial grade units with an approved testing agency seal. Inspector's comments: Facility is clean.

Willard Drive-Thru; 1052 Myrtle Ave., Willard; operator, Raymond Secor (June 15, regular inspection) Violation: Person-in-charge must be knowledgeable about food rules and procedures; Violation: Food employees must wear hats/hairnets when preparing/serving bratwurst; Violation: Food employees must use gloves, tongs, napkins, or other methods to handle ready-to-eat food. This includes sandwich buns and various condiments.

Wendy's of Willard; 114 Walton Blvd., Willard; operator, Wenco Wooster, Inc. (June 18, regular inspection) Violation: Make sure that all pans are air-dried before returning to storage rack. Inspector's comments: Facility neat and clean. Note: Front reach-in refrigerator was not keeping temperature at this inspection. Manager had already called for repair work. Butter, sour cream, and cheddar cheese (for chili) was moved to another refrigerated unit.

Green Witch Suds & Sundaes; 13 E. Main St., Greenwich; operator, Michael and Darlene Meehan (June 19, regular inspection) Violation: Several pieces of equipment not commercial grade. Remove or replace with units that have been approve by a recognized food testing agency; Violation: Food employees not wearing hair restraints at inspection. Wear proper hair restraints while in food prep area. Inspector's comments: Facility very clean and in good condition.

Charlie's Bar; 54 Benedict Ave., Norwalk; operator, Donna Rossman (June 22, regular inspection) Violation: Install screen door if outer doors are opened in warmer weather for ventilation purposes. Inspector's comments: Facility is in good shape at this inspection. Note: If switching to chlorine sanitizer, remember to have chlorine test strips.

Pizza Brothers; 210 Milan Ave., Norwalk; operator, David Allison (June 22, regular inspection) Violation: Person-in-charge must be knowledgeable about food rules and procedures; Violation: Garlic butter contains cheese and needs to be kept refrigerated. Check with supplier to find best way to use this product. Inspector's comments: Facility neat and clean.

Willard Pool; 561 W. Laurel St., Willard; operator, Dennis R. Hammond, City of Willard (June 25, regular inspection) Violation: In three compartment sink, prepare wash, rince and sanitize in correct sequence.

Victory Inn; 6 Myrtle Ave., Willard; operator, Brian Hamman (June 28, regular inspection) Violation: Repair toilet in men's restroom; Violation: Provide soap in men's restroom.

Bruno's Pizza; 11 W. Maple St., Willard; operator, Bruno's Pizzeria LLC (June 29, regular inspection) Violation: Provide new test strips. Old ones have been saturated and are no longer good; Violation: Fluorescent lights that have been added need protective shields; Violation: Cover drywall in pizza making area. Walls need to be easily cleanable and nonabsorbent. This will need to be done within 30 days.
